• Enrollment Forms

    The following forms will be required for enrollment with your student's information.  You may print them out, complete them by hand, and return them to CSL by mail, in person, or scanned and emailed to admissions@cslcharter.org.   You can also fill them out on the computer, save them, and email them as attachments to admissions@cslcharter.org. If you have any questions while filling out the enrollment forms, please email admissions@cslcharter.org or call 215.269.7390 for assistance. 


  • Other Required Documents

    In addition to the forms above, CSL will also need the following information to complete the enrollment process: 

    • Student's Birth Certificate
    • Student's Social Security Card
    • 2 Documents Proving Residency (i.e. PECO bill, cable bill, water bill, mortgage/lease, etc.)
    • Student's Immunization Records
    • Copy of the most recent Individualized Education Plan (IEP) and Reevaluation Report (if applicable)
